Aldolase Blood Test (Muscle Enzyme)

Aldolase is an enzyme found predominantly in skeletal muscle, and to a lesser extent in the liver, brain and red blood cells. When muscle fibres are damaged or inflamed, aldolase leaks into the bloodstream, making it a useful marker for detecting and monitoring muscle injury. This test is particularly valuable when creatine kinase (CK) results are normal or inconclusive, as aldolase can be elevated in certain myopathies and inflammatory muscle conditions where CK is not. It is also used to help investigate unexplained muscle pain, weakness, or suspected muscle-wasting disorders.

This test is suitable for patients presenting with persistent muscle pain, stiffness or weakness, those with suspected inflammatory myopathies (such as polymyositis or dermatomyositis), and individuals being monitored for muscle-related side effects of certain medications (for example statins). It may also be requested alongside other muscle and liver enzymes to build a clearer clinical picture.

The test requires a simple blood sample, collected by a trained phlebotomist. No special preparation is usually needed, although your doctor may advise avoiding strenuous exercise for 24 to 48 hours before the sample is taken, as intense physical activity can temporarily raise aldolase levels.

Why it matters
Aldolase provides a direct window into muscle health. While CK is the more commonly used first-line muscle enzyme, aldolase can be elevated in some conditions where CK remains normal, helping to avoid missed diagnoses. It is also useful for tracking disease activity over time and assessing response to treatment in inflammatory muscle diseases. By measuring aldolase, your doctor can better determine whether muscle symptoms are due to injury, inflammation, or another underlying cause, guiding appropriate further investigation and management.
