Anti-Mitochondrial Antibody Blood Test (AMA)

The Anti-Mitochondrial Antibody (AMA) blood test is used to detect the presence of anti-mitochondrial antibodies, which are autoantibodies directed against components of the mitochondria within cells. This test is primarily requested when a clinician suspects primary biliary cholangitis (PBC), a chronic autoimmune liver condition that causes progressive destruction of the small bile ducts. It may also be used to investigate unexplained cholestatic liver enzyme abnormalities, such as raised alkaline phosphatase (ALP), or to help differentiate PBC from other forms of liver disease. The test is suitable for adults presenting with symptoms such as fatigue, pruritus, or jaundice, as well as for monitoring individuals with known autoimmune liver disease.

The analytical method used for this test is typically indirect immunofluorescence (IIF) or enzyme-linked immunosorbent assay (ELISA), both of which are well-established techniques for detecting specific autoantibodies in serum.

Why it matters
Anti-mitochondrial antibodies are a highly specific marker for primary biliary cholangitis, present in approximately 95% of cases. Early detection allows for timely intervention with ursodeoxycholic acid, which can slow disease progression and reduce the need for liver transplantation. A positive result, in the appropriate clinical context, provides strong evidence for PBC and helps guide further management, while a negative result may prompt consideration of other causes of cholestatic liver disease. This test therefore plays a critical role in the diagnostic pathway for autoimmune liver disorders.
