Cardiac Troponin I (cTnI)

Cardiac troponin I (cTnI) is a protein released into the blood when the heart muscle is damaged. It is one of the most specific and sensitive markers of heart muscle injury available, which makes it a key test in assessing the health of the heart. Because troponin is found almost exclusively in heart muscle, a raised level is a strong signal that the heart muscle itself has been affected.

What it is used for

• Detecting heart muscle damage: troponin rises when heart muscle cells are injured, for example during a heart attack
• Assessing chest pain and related symptoms: it is a central test used by clinicians to investigate suspected heart problems
• Monitoring the heart: it can be used to help assess strain or injury to the heart muscle in a range of situations

An important note on interpretation

Cardiac troponin reflects the current condition of your heart muscle rather than your long-term risk of heart disease. A normal result is reassuring for that moment but does not predict future risk, and a raised result always needs prompt medical review to understand the cause. Troponin can also rise for reasons other than a heart attack, including other heart and non-heart conditions, so the result is always interpreted alongside your symptoms and other clinical information.
