Free Prostate-Specific Antigen (fPSA) is a specialised blood test used to refine prostate cancer risk assessment, particularly when total PSA is elevated but a digital rectal examination is normal. It measures the proportion of PSA that circulates unbound to proteins, helping clinicians distinguish between benign prostate conditions and prostate cancer. This test is typically requested for men with a total PSA between 4 and 10 ng/mL, where the free-to-total PSA ratio can improve diagnostic accuracy and reduce unnecessary biopsies.
The test is performed using a simple blood sample, with analysis conducted via immunoassay techniques. It is not a standalone screening tool but is used alongside total PSA and clinical findings to guide decision-making.
