Insulin Autoantibody Blood Test (IAA) – Type 1 Diabetes

The Insulin Autoantibody (IAA) blood test is used to detect autoantibodies directed against insulin, which are a hallmark of autoimmune (Type 1) diabetes. This test is particularly valuable in the early stages of Type 1 diabetes, often before the onset of clinical symptoms, and is frequently used in the diagnostic workup of individuals at risk, including those with a family history of Type 1 diabetes, those with other autoimmune conditions, and in the investigation of hypoglycaemia in non-diabetic patients. It is also a key component in the prediction and monitoring of disease progression in research and clinical settings.

This test measures the presence and level of insulin autoantibodies (IAA) in the blood. IAA are autoantibodies that bind to endogenous insulin. They are one of the four major islet autoantibodies used to assess autoimmune diabetes risk, alongside GAD, IA-2, and ZnT8. The presence of IAA is most common in young children who develop Type 1 diabetes, and it often appears as the first autoantibody in the preclinical phase. The test is performed using a validated radioimmunoassay or enzyme-linked immunosorbent assay (ELISA) methodology.

Why it matters
Identifying IAA can help confirm an autoimmune aetiology in patients with new-onset diabetes, distinguish Type 1 from Type 2 diabetes, and predict disease progression in at-risk individuals. Early detection of IAA, especially in combination with other islet autoantibodies, allows for timely intervention, monitoring, and enrolment in prevention trials. For patients with atypical presentations or suspected autoimmune hypoglycaemia, IAA testing provides critical diagnostic clarity.
