The Lp-PLA2 (PLAC) Test measures the activity of lipoprotein-associated phospholipase A2, an enzyme produced by inflammatory cells within atherosclerotic plaques. Unlike cholesterol tests, which quantify lipids, this test provides a direct window into the inflammatory processes that drive plaque instability and rupture. It is particularly valuable for adults with a family history of premature cardiovascular disease, those with intermediate traditional risk scores where clinical decisions remain uncertain, and individuals who have already experienced a cardiovascular event but show normal standard lipid profiles. The test is performed on a simple blood sample using an enzyme-linked immunosorbent assay (ELISA) or similar immunoassay technique to quantify Lp-PLA2 mass or activity.
What it screens for
- Lp-PLA2 (PLAC): This enzyme is predominantly bound to low-density lipoprotein (LDL) and is a specific marker of vascular inflammation. Elevated levels indicate active inflammation within arterial walls, which is associated with an increased risk of coronary heart disease, ischaemic stroke, and cardiovascular events. Unlike high-sensitivity C-reactive protein (hs-CRP), which reflects systemic inflammation, Lp-PLA2 offers a more targeted assessment of plaque-specific inflammatory activity.
