This profile investigates the hormonal factors behind excess or unwanted hair growth in women, known medically as hirsutism. Excess hair, particularly in a male pattern such as on the face, chest, or back, is often linked to raised levels of androgens (male hormones, which women also produce in small amounts). By measuring the key hormones involved, this profile helps identify a hormonal cause, such as polycystic ovary syndrome (PCOS), which is the most common reason for excess hair growth.

Androgens

These hormones are the main focus, as raised levels are the usual cause of excess hair growth:

  • Testosterone: the main androgen. Raised levels can drive excess hair growth
  • Sex hormone binding globulin (SHBG): a protein that binds testosterone, used together with testosterone to work out how much is actually active
  • Free androgen index (FAI): a calculated measure of your active androgen levels, often the most useful single indicator in investigating excess hair

Reproductive hormones

  • Luteinising hormone (LH) and follicle-stimulating hormone (FSH): the balance between them can help identify PCOS, a common cause of both excess hair and irregular periods
  • Prolactin (PRL): a hormone that, when raised, can disrupt other hormones and contribute to hormonal symptoms

Thyroid

  • Free thyroxine (fT4): checks thyroid function, as a thyroid imbalance can contribute to hormonal symptoms and changes in hair

These hormones are best understood together and alongside your symptoms. Several of them, particularly LH, FSH, and testosterone, vary across the menstrual cycle, so the timing of the test matters, and testosterone is best measured in the morning.
