Renin Blood Test (Active Renin)

The direct renin blood test measures the concentration of active renin, an enzyme released by the kidneys that plays a central role in regulating blood pressure and fluid balance. Renin initiates the renin-angiotensin-aldosterone system (RAAS), a hormonal cascade that controls blood vessel tone and sodium retention. When this system is overactive or dysregulated, it can drive persistent hypertension, making renin a key marker in the investigation of secondary and resistant high blood pressure.

This test is typically requested for patients with difficult-to-control hypertension, early-onset hypertension, or hypertension accompanied by low potassium, to help distinguish between primary aldosteronism and other forms of secondary hypertension. It is also used to monitor the effectiveness of RAAS-blocking medications such as ACE inhibitors, angiotensin receptor blockers, and direct renin inhibitors. The analysis is performed using a validated immunoassay technique that provides a precise measurement of active renin.

Why it matters
Renin is the rate-limiting step in the RAAS pathway, so measuring it directly gives clinicians a clear window into whether the kidneys are over- or under-driving blood pressure. An elevated renin level may point to renovascular disease, renin-secreting tumours, or the effects of certain antihypertensive drugs, while a suppressed renin level can support a diagnosis of primary aldosteronism or other low-renin hypertension. By identifying the underlying hormonal driver, this test helps guide targeted treatment, avoid ineffective medication trials, and reduce long-term cardiovascular and renal risk.
